Letter of Transmittal

Does anybody here have any experience with filing a Letter of Transmittal for a stock certificate? 

I own shares of Genta, a cancer research company.  They just reverse split and changed their ticker from GNTA to GETA.  According to their website, I will receive a letter in the mail with instructions on filing for transmittal.  Unfortunately, it takes a while for my mail to get forwarded overseas. 

I was hoping my brokerage would handle this, but apparently not.  Anyone esle ever have to go through this process?  Any advice or suggestions?
